- Combat: The new targeting system and cover mechanics (inspired by Gears of War) were revolutionary for the series. Gunfights became tense, strategic battles of positioning rather than run-and-gun spam. The Euphoria physics engine made enemies react dynamically to bullet impacts—stumbling, grasping wounds, and clutching onto ledges.
- Driving: Perhaps the most controversial change. Cars had heavy, realistic weight and suspension. They rolled, pitched, and required braking and skill to handle. While arcade fans balked, many praised the tactile, visceral feeling of wrestling a muscle car through a rainy Liberty City night.
- Police System: The wanted system was overhauled. To lose the cops, you had to break their line of sight and stay out of their search radius, which expanded and contracted realistically. This led to thrilling, cat-and-mouse chases through alleys and tunnels.
- Side Activities: Beyond the main story, Niko could go bowling, drink, play darts, attend cabaret shows, or race. While criticized by some as "errand boy" tasks, these activities deepened character relationships (unlocking special abilities, like Little Jacob's mobile weapon delivery).
